What is the definition of “breast implant alternatives”?
Breast Implant Alternatives are a range of procedures that are intended to reconstruct, lift, or enlarge breasts after removal of breast implants. Implants are removed for many reasons. They may no longer ‘fit’ your lifestyle or body-image. The breasts may enlarge with age, menopause, and weight gain so that breast size with an implant may now be larger than the original size when the implants were inserted. Implants can result in one or more of the following conditions: capsular contracture, implant rupture, asymmetry, or discomfort. Another reason for removing implants is the presence of a textured implant and concerns about the risk of developing BIA-ALCL. Breast Implant Illness is a poorly defined condition but may also be a reason for implant removal. Sometimes patients simply want to have their implants removed. Surgery can include removal of the implants and capsule removal (capsulectomy).
If implants are removed without replacement, a breast auto-augmentation (BAA) is performed simultaneously. This is called a Simultaneous Salvage Auto-Augmentation or SSAA. Breast enlargement using the patient’s own fat may also be done at the same time as SSAA. The fat is harvested from the thighs, hips or tummy and then processed and reinjected into the breasts at all tissue levels such behind and in front of the muscle behind the breast and in the subcutaneous fat layer.

Surgical technique
There are multiple techniques, which may be combined:
1. Breast implant removal with partial or complete (en-bloc) capsule removal
2. Breast Auto-augmentation with breast lift (mastopexy).
3. Simultaneous Salvage Auto-Augmentation (SSAA): (1) & (3) combined.

Philosophy
To create a soft natural-feeling breast that is perky or teardrop-shaped, with a ‘lollipop’ scar that turns white and is almost unnoticeable when completely healed.
Surgical time and recovery
Surgery takes about 3 hours. Fat grafting to the breast adds an additional hour. There are no drains unless a complete or significant capsulectomy is performed at the same time. Most patients experience minimal discomfort and are ambulating the same day. Patients can generally return to light work after 10 to 14 days.
