Not all women wish to improve the appearance of their breasts with implants. In fact, women with existing breast implants may want to have them removed. Additionally, implants are not “forever” devices. Due to complications that can arise over time, they may generally need to be removed or exchanged every 10 to 15 years. As with all procedures available at Kirwan Plastic Surgery, the area’s most experienced surgeon performs advanced alternatives for those patients who desire augmentation without implants.
Dr. Laurence Kirwan performs the Simultaneous Areolar Mastopexy Breast Augmentation (SAMBA™) procedure. While this may sound like a mouthful, the procedure generally lifts breasts that have sagged. So, we can raise the nipple between two centimeters (.8 inches) above and two centimeters below the inframammary crease under the breast. SAMBA™ treatment involves the use of an implant.

We may perform areolar mastopexy alone in cases where patients either do not need or do desire implants. The process of lifting the breast tissue by removing skin around the areola is appropriate when the nipple is at or above the inframammary crease. We can use other techniques for more severe cases of breast ptosis or drooping, such as a vertical mastopexy. This approach involves making a lollipop-shaped incision around the areola that moves vertically, downward toward the folds of the breast. The areolae and nipples are lifted to a higher position.
SAMBA™ is most suitable for women with smaller breasts. It may also be appropriate for those with less severe laxity. The nipple should not fall below the inframammary crease. For those with bigger breasts and more severe laxity, Dr. Kirwan may discuss a breast lift with skin excision (removal) that may or may not include removing tissue in the lower pole of the breast below the areola. This technique results in the aforementioned vertical, lollipop-shaped incision/scar. It may also involve a scar along the crease in the shape of an anchor or inverted “T.” Put another way, this latter inverted-T or wise pattern technique involves three incisions around the areola, vertically down the inframammary fold, and along the fold. This results in the anchor or “T” shape. Through the incisions, excess skin is removed and the breast is reshaped.
A breast lift may be performed at the same time as breast implant removal. It can result in highly satisfying results as the breasts are repositioned higher on the chest and excess skin is tightened. In fact, the presence of implants can contribute to sagging that occurs over time. You may no longer like the look of your implants, as they feel too heavy and uncomfortable. You may feel scar tissue is harder and tighter, or imaging may suggest a leak or rupture. Whatever the case may be, implant removal is generally performed through incisions along the inframammary fold or around or below the areola. Through Dr. Kirwan’s expert techniques, the capsules and implants are removed, and the incisions are closed.
